FAS Student Records Request Form

For students enrolled in our High School Diploma program, all high school transcripts are needed upon enrollment in order to ensure appropriate course placement. The FAS Student Records Request Form needs to be dated and signed by the adult student (as highlighted and shown in the form below) and submitted to the FAS office located at:
